SAT/ACT Fee Waiver Request

**You must submit this form for each fee waiver you are requesting.

Juniors and Seniors who qualify may request up to (2) SAT,  (2) SAT subject test, and (2) ACT fee waivers.

Students are limited to (2) SAT, (2) SAT subject test, and (2) ACT fee waivers for their total time in high school. If you receive your all your fee waivers all in the Junior year, you will NOT receive any fee waivers Senior year.

Special note for ACT fee waivers- these expire on August 31st, 2018. Please be sure to use your ACT fee waiver before this date.
