Membership Cancellation Form
*Your request must be filled out by the 1st of the month in order for your cancellation to occur the following month (a minimum of 30 days notice is required).
*All cancellations occur on the 1st of the month to coincide with our billing cycle (the one exception to this is if you are moving out of the area mid-month)
*We will not go back and credit dues once they have been paid. All requests are for future months only.
*Once you cancel your membership, should you decide to rejoin, you will be charged half the current registration fee.
