Faith Academy Facilities Use Request

Please allow TEN business days to process your request and confirm if the date and facility you are interested in is available for your event. Completing this form does NOT guarantee approval/confirmation for your event. You will receive an email with confirmation status.

All facilities - A minimum refundable deposit of P2,000 may be required depending on size of group. Facilities must be left clean and undamaged. Please allow 5-7 business days for facility inspection and refund to be processed. A full refund will be issued if the event is canceled one week in advance. If an event is cancelled less than one calendar week in advance 75% of the fees of the event will be refunded, within 7 business days.

If you are requesting both a non-Cadd space AND a Cadd space you may include the Cadd space in the "other" section of your request.

If a Faith sponsored event, please provide the account number for the department that should be charged if damages occur. Deposit fees will not be charged if FA account number is provided. If non-Faith sponsored and you do not have or do not wish it use a Faith Academy account type NONE. Full deposit fees will be applied. If applicable, please indicate the start and end time you wish to use the pool. How many will swim? We must arrange for 1 lifeguard for every 20 people. Pool use is dependent on lifeguard availability and weather permitting. Pool passes are required to be purchased prior to the event for non-Faith members.
