Accuplacer Request Form
Hello EOU Student,
Please answer all questions and submit the information so we may generate a voucher number for you to take your Accuplacer placement test.  If you are in Union Co., we can schedule you to test on campus (M-F/8am-3pm).  If you are out of Union Co, we can schedule a proctoring session via Zoom meeting.  If you have a school issued Google Chrome book, it will not work with Zoom because of the school’s restrictions to keep you from downloading on it.  
Prior to taking the Accuplacer, we encourage you to view the Accuplacer Study App (https://accuplacerpractice.collegeboard.org/login) to assure yourself the best score you can obtain during your placement test. Here you will find study material/practice questions and feedback on your testing performance.  If you have any questions, or would like to test on campus at the EOU Testing Center, please contact us at testing@eou.edu.
