Embellished Bayou Order Form
Use this form to request an order with Embellished Bayou.  Applique shirts are $21 for short sleeve and $23 for long sleeve (add $3 for personalization), plus shipping and 5% LA tax (if applicable). Personalization
Type name or monogram exactly as it should appear (optional, adds $3 to shirt price). Girl monograms are typically first name initial, LAST name initial, middle name initial with last name initial larger in the middle. Boy initials are typically embroidered in first, middle, last initial order all with letters the same size.
