Illinois law mandates that schools provide age-appropriate sexual abuse and assault awareness and prevention education to students in Early Childhood through Grade 12.

Please complete and sign this form ONLY if you do NOT want your child to participate in this program this year.

This form must be received electronically no later than Thursday, February 5th, 2015. If a parent does not submit this opt-out form for a student by that date, the student will participate in this program.
