Complete the following form with all requested information to ensure that your transcript will be sent out. A transcript includes your GPA, all courses taken in high school and test scores. THIS FORM MUST BE COMPLETED FOR EACH SCHOOL OR SCHOLARSHIP REQUESTING A HIGH SCHOOL TRANSCRIPT.

Each college application/scholarship application will tell you exactly what is required.

It is the student’s responsibility to contact the school to verify information was received by the college.
