Instructions
All information on this form must be completed and received no later than June 1.
The following information should be included, at the time of nomination, if possible:
1) Photocopies of newspaper clippings, articles, event programs, or other such material that
detail information of the nominee’s accomplishments. Please do not send originals as we are
unable to return these items.
2) A photograph of the nominee will be required should they be selected.
3) All information submitted shall be retained by the Hall of Fame Committee, and all action
necessary to the selection process shall remain confidential.
