High School Transcript Request Form
A transcript is a copy of a student’s permanent academic record. A transcript may also contain personally identifiable and directory information such as date and place of birth, high school graduation date, etc. Thus, a transcript is only released upon prior consent of the student, except as provided by law.

Directions:
Please provide the following information to assist us with your transcript request. Transcript processing time is 3-5 business days after your request is received. Transcripts sent directly to a student or as a PDF sent electronically via e-mail are marked “unofficial.” Please note that a third party may not accept these transcripts.
