Submitting this form completely and accurately constitutes the written notification required as per membership agreement: "I understand that my membership is continuous until 60 days after written notification has been received by the Dad's Club Swim Team Treasurer."

Monthly billing dates for facility membership are on the 15th of every month while all other programs are billed on the 1st. Membership fees cannot be prorated.

For example, if you cancel a swim team membership on June 10, then this membership ends effectively on August 31 with the last monthly bill issued on August 1.
