This form can also be used to request full-time enrollment status verification for insurance companies, scholarships or for Social Security or VA purposes.
All fees must be paid in full before transcripts will be produced.
IMPORTANT: If, for any reason, a request for records cannot be completed as requested, a CSEC staff member will contact the person requesting the records giving the reason and the course of action needed. If the reason is due to a PowerSchool issue or some other reason outside of the student's control, CSEC staff will make it a priority to resolve the issue quickly, and will produce the records as soon as possible. If the reason is based on an action the student needs to take, the request will NOT be saved. After the issue has been resolved, the student must place a new records request.
