CSD Film Festival Entry Form

This is the official entry form for the 8th Annual Canyons District Film Festival.
This form must be completed and submitted no later than 5:00 pm on March 23, 2017.
All entries must be submitted electronically this year. No paper entry forms, disks, or drives will be accepted.

    Film Festival Information

    •Submit a separate entry form for each film/poster you are entering. •You must submit your entry using this Google Form. No paper forms will be accepted. •You must submit your entry using your CSD Docs account and include the URL to your film/poster on this form. Films must be in one of the following file formats: .mov, .mp4, .m4v. Posters must be in one of the following file formats: .jpg, .png, .pdf. •By submitting your entry, understand that it might be shared by Canyons School District via the Internet and other Festival-related communications. It might also be shared through various news and television outlets, including the Utah Education and Telehealth Network (http://uetn.org/), which sponsors our American Graduate Film Festival strand. •You must verify that each person involved in your entry has permission to have their name/likeness posted online. People who have not accepted the Canyons District Uniform Media Release must complete a Canyons School District Information Release form (http://goo.gl/iQfljZ). It is your responsibility to collect and keep these forms. You may be asked to hand these forms over to Canyons School District if your film is selected as a winner. •You must verify that you own the rights to all media contained in your entry. If you are using royalty-free materials, you must include credits for these materials in your entry. If you are using copyrighted materials, you must be able to provide written permission for your use of the material from the copyright owner. •Film Festival entries that break copyright, do not contain proper credits, or do not have proper media and information releases for each person involved in the film will not be considered for Canyons District Film Festival awards. •Films that go over the time limit for the chosen category will not be considered for Film Festival awards. •Films that are too violent or graphic in nature will not be considered for Film Festival awards.
