2016-17 Burley Out of School Time - Club Request Form

Club requests are due by Friday, December 23rd. Complete this form once per child.

This form does not guarantee a spot in any club, as space is limited. If you need assistance completing this form, please call the school office at (773) 534-5475.

If your child is admitted to a club, you will receive a confirmation form and invoice the week of 1/9. There is a cash fee of $50.00 for each Burley Out of School Time Club for which your child is confirmed (Running Club, Sustainability, and Geometry fee is $75.00).

Do not send money until you receive your confirmation and invoice.

Please be sure not to request clubs on days when your child will be attending a vendor-run club or an academic club. See the At A Glance Club Schedule.
