To request a Costco logo, complete this form and provide a detailed description of your intended logo use. Logo use permission is granted on a case-by-case basis. Approvals are for one-time use only; subsequent use of the Costco logo requires a separate request to be submitted.
If you are a current vendor for Costco and are requesting the Costco logo to promote a product on your website or other advertising opportunity, please note that additional levels of approval will be required. Please check with your assigned Costco Buyer first before submitting this logo request form. They in turn will need to obtain approval from their GMM (General Merchandise Manager) and/or Executive VP before they can authorize use of the logo from us.
If you are a Warehouse Employee:
1. All materials that will be seen by members or by the general public must be created by the Corporate Communications and Marketing Department. 4. Yellow Pages Advertising - It is Costco policy to participate in Yellow Page advertising as a line listing only.
