Transcript Request Form
Using this form, you may request a copy of your Columbia High School transcript.

Official transcripts may be mailed to the location or school of your choice or picked up in the CHS office during regular office hours. Emailed transcripts may not be accepted by all colleges or universities as official.

If you are requesting your transcript be sent to multiple locations, you MUST make a request for each location. Depending on how you want your transcript sent, you MUST provide an accurate and complete email address OR mailing address. Email addresses and mailing addresses will NOT be provided by the CHS office.

Requests will not be processed if the information provided is not complete.

Requests may take up to 48 hours to be processed and will only be completed during regular office hours.
