Transcript Request Form

To request a copy of your Columbia High School transcript, you must complete this form.

Official transcripts may be mailed to the location or school of your choice.
Please note that emailed transcripts may not be accepted by all colleges or universities as official.

If you are requesting your transcript be sent to multiple locations, you MUST make a request for each location.  Depending on how you want your transcript sent, you MUST provide an accurate and complete email address OR mailing address.  Email addresses and mailing addresses will NOT be provided by the CHS office.

Requests will not be processed if the information provided is not complete.
