FERPA Directory Information: Non-Disclosure Form
Higher education institutions are allowed to publish “directory information” including the student’s name, address, telephone number, date and place of birth, program of study, participation in officially recognized activities and sports, weight and height of members of athletic teams, dates of attendance, degrees and awards received, and the most recent previous educational institution attended.

If any current CCU student does not want such directory information to be disclosed, he or she must notify Student Services of the specific information not to be released by completing the FERPA Directory Information: Non-Disclosure Form.

Notification is necessary within ten days of the first day of class of the fall semester annually.
