PROJECT HOPE Application Form 2013-2014

Hello Volunteers!

Welcome to Boston University Community Service Center. Project Hope is dedicated to raise awareness about public health issues in the Boston University Community and the city of Boston. We focus on issues related to the Human Immunodeficiency Virus (HIV), the Acquired Immunodeficiency Syndrome (AIDS), mental and physical health. We are dedicated to the assistance to those affected by HIV/AIDS and sexual health related problems. In addition, Project Hope is committed to support public safety and public health education throughout the Boston area and beyond. The time commitment varies but it usually is 2-3 hours a week.

    **If you participated in FYSOP as a staff, you do not need to complete these steps.** By clicking agree, you agree to completing the following steps: -Go to http://www.bu.edu/csc/opportunities/community-service-center-programs/project-hope/ -Print the 4 PDF documents on that page, SINGLE SIDED. -Fill out the forms, and DO NOT staple. -Return these forms to the CSC on the 4th floor of the George Sherman Union ASAP, anytime between 9am and 5pm, Monday-Friday. Please bring a valid photo I.D. (driver's license, passport, etc.)
