Parent Portal Access Form
In order to protect you and your students personal information, the following steps are required for your Parent Portal account to be created:

1) Complete all fields in this form and submit the form.
2) Once you have submitted the online form, you will need to come to the school Front Office to provide photo identification to verify your identity.
3) You must be listed as an authorized parent/guardian on your student's records in order to have access to Parent Portal

Your Parent Portal account will not be created until both steps are completed. This is to ensure that only designated parents/guardians have access to their child(ren)'s information. Thank you for your cooperation!
