Crowdfunding Policy #815.7 - Approval Form
If you are interested in raising funds or requesting items via a crowdfunding site (e.g. www.gofundme.com, www.donorschoose.org, etc.), you are required to complete this pre-approval form. This a requirement of BASD school board approved Policy #815.7, so there are no exceptions.

You are not allowed to proceed with posting anything on a crowdfunding site until you have received written approval of this completed form.

Please review the questions/information requested on this approval form to ensure that you have all of the required information ready. If you do not have all information requested in this form, please locate it/write it before completing this approval form.

Requests submitted from individual teachers will be reviewed and approved by Dr. Jodi Frankelli, Supervisor of Grants (ext. 60228). Do not proceed until you receive approval.

Requests submitted from clubs/organizations will be reviewed and approved by the building principal.
