SM AYSO TEAM VOLUNTEERS - U12 2013
Please complete the following form to help us match your team's volunteers with our programs and events.  Each family is expected to volunteer at least 1 hour per week during the season.  There is a role for each family on your roster below.

Note:  All teams will still be expected to provide 2 workers for a 2 hour shift at Picnic in the Park Day, but that in and of itself is not a volunteer role.

In rare instances, due to work commitments families can make a donation of $250 to help the organization rather than commit to a volunteer role.  Please note OPT-OUT in the fields above for the unfulfilled role, and note the parent's info below so we can confirm with the treasurer that they have made the donation.
