Complete the survey using the following guidelines:

1. Report only whole numbers, not ranges. For example, if there were 20-25 ASPIRE activities please report it as 20 (or 25, as the case may be).

2. Reports are due January 15 (September-December) and May 15 (January-May)

3. Year-to-date totals: numbers will increase each reporting period or stay the same.

Student Involvement

Students Enrolled: Number of students in each grade level with signed ASPIRE permission forms on file.

Other ASPIRE-related Activities: Number of students participating in ASPIRE-related activities. Count total participants, for example, if one student

attended three workshops, report the number as “3.” The following examples can be counted as Other ASPIRE-related Activities.

This is a duplicated headcount:

Number of times that non-ASPIRE students received drop-in help from the ASPIRE Center.

Number of students who attended an ASPIRE workshop (in or out of the classroom).

Number of students who received other college access information and/or assistance due to ASPIRE, such as attending a college fair, a classroom activity

or a campus visitation.

Volunteer Involvement

ASPIRE Volunteers: Number of participating ASPIRE volunteers.

Traditional Volunteer: An adult volunteer, eighteen years or older, recruited and screened by ASPIRE sites.

Episodic Volunteer: An adult volunteer, eighteen years or older, who leads college and career focused presentations and activities for groups of students in the presence of site staff.

Peer Mentor: An ASPIRE high school junior or senior who has participated in the ASPIRE peer mentor model.
