APEX Premium Member Information Update
Hello Premium Member!

1. Go to www.apex.org/premium/login to complete the update.  If you have not logged onto the new site before, click on "Forgot your password?" and follow the instructions to reset your password.

2. After resetting your password, click on "My Profile", on the left side navigation bar to verify and update your profile.

3. Once you have updated ALL of the fields, complete the form below and click submit.  You will receive an email from one of our team members with your coupon code for your FREE year of membership or that you need to make corrections before receiving the coupon.  The fields are:

- First & Last Name
- Gender
- Industry
- Zip Code of Residence
- Company Name
- Job Title
- Education Level
- Birth Date

Instructions for redeeming the coupon will be in the e-mail with the coupon code.  You must complete your profile update BY JANUARY 31, 2013 to be eligible for the free year of membership.
