Applications are due Tuesday, April 16th by 11:59pm.
A description of the group being nominated and why this student organization
deserves the Community Service honor by specifically addressing the criteria below
Two recommendation letters from any of the following: students, faculty/staff, community members,
or anyone else who is familiar with the group and/or the service project. It is encouraged,
but not required, to provide one letter from those who were impacted by the service project.
Additional supporting materials that are pertinent to the nomination are encouraged,
but not required. This could include newspaper articles, photos, reflection pieces,
budget worksheets, records of attendance, training certification papers, etc.

The project for which the group is being recognized must have taken place between
April 19, 2012 and April 15, 2013.
The service must be shown to have made a positive impact on both the community
and the student organization. The service should be both thoughtful and
meaningful. Please answer the following questions to illustrate the impact:
o What community need was addressed?
o How were volunteers educated about this need?
o How did this service make a difference or impact the community?
o How did this service impact the student organization?
The service must have been done in the spirit of volunteerism and should not have
been done in return for monetary compensation.
