2016 TOYM Nomination Form

The Ten Outstanding Young Minnesotans program is Minnesota’s only statewide recognition program for outstanding young leaders. The program acknowledges the efforts of individuals between the ages of 18 and 40 that have contributed to our state through their service, thought and influence, community involvement, or entrepreneurship.

Thank you for your interest in submitting a nominee for this important recognition. You can email Debbie Ackerman at toym@mnjaycees.org if you have any questions about the nomination process. Once your nomination has been submitted, we will confirm accuracy of the information submitted with the nominee.

To complete the nomination form you will need to provide the following information. Please have all your information before you start the form - you will not have the ability to save it and come back later. LinkedIn, resume, and co-workers/co-volunteers are all helpful sources for information about your nominee. We will follow up with you for any additional information needed about the nominee. Information for Nomination:

-Education (Degrees, Academic Honors, etc.)
-Outline the timeline of the nominee's career or field of activity
-Details on nominee achievements and/or challenges they had to overcome to achieve them
-Important awards and honors that illustrate their achievements and brief description of each
-Quotes from authorities that illustrate their achievements
-How the nominee exemplifies "service to humanity is the best work of life"
-Why you think your nominee should be selected

CATEGORIES

1. Business, economic, agricultural and/or entrepreneurial accomplishment
2. Political, legal and/or governmental affairs
3. Academic leadership and/or accomplishment
4. Cultural achievement (to include literature, history, education or all arts)
5. Moral and/or environmental leadership
6. Contribution to children, world peace, and/or human rights
7. Humanitarian and/or voluntary leadership
8. Scientific and/or technological development
9. Personal improvement and/or accomplishment
10. Medical innovation


PROGRAM RULES

-Nominees must be between the ages of 18-40 and can only be nominated in one of the ten categories.

-Nominees do not need to be a member of the JCI organization.

-The nominee must be a native-born or naturalized citizen of the United States or must have applied for citizenship by January 1 of the preceding year.

-The nominee may not be a current member of the JCI Board of Directors at the International, National, State or Local levels. In addition paid JCI staff members will not be considered for the TOYM program.

-All information must be contained in this nomination form. No attachments of any kind are allowed. If a candidate is selected as one of the honorees, additional items will be requested by JCI Minnesota, such as pictures for PR and event slideshow.

-There is a $25 application fee per submission. Please send payment to: JCI Minnesota / 5775 Wayzata Blvd, Suite 700 / St Louis Park, MN 55416 by the July 1 deadline.

-If your nominee is chosen as one of the 10 honorees, an additional $125 honoree fee will be required.

-Nomination forms must be submitted by JULY 1, 2016
