Burlingame Soccer Club Financial Aid Application
Each season Burlingame Soccer Club (BSC) has limited Financial Aid funds available. Financial aid is awarded to players based on financial need. The amount of funds awarded each season is directly dependent on the amount of funds available and the number of applications received.

Step 1: Complete and submit the electronic form below for each player applying;
Step 2: Provide the following, preferably by email:

- Current and Prior Year Federal Form 1040 Tax Return (pages 1 and 2) and Schedule C (if applicable) for both parents/guardians*;
- Recent (last 2) copies of paystubs for all working adults in the household*;
- Any other documentation that might help the Financial Aid Committee understand your need.

If you cannot email this information, please mail all required documentation by the application deadline and include the cover page to:

If parents are not together, both tax returns must be submitted and if both parents are not listed on the tax return, a request for the other tax return will be made.
