Payment:Business Vendors: Please attach a check for $30 payable to “Our 42nd Avenue” and indicate that the monies go to Cully Farmers Market. Send Check and application to Our 42nd Avenue - 5140A NE 42nd Ave. St. Portland, OR 97218 *Registration is not valid until fees are paid.
By signing below, I acknowledge responsibility for:Proper licensing/equipment - Self-supplied tents, tables, chairs, etc. & insurance * Canopy weights required
Self-supplied electrical access - Loss of vendor fee for cancellation Self-supplied containers for water within 10 days of the event
And that I will accept community tokens for hot and prepared foods, which are redeemable for a check for their total face-value at the conclusion of the event.
