2020 Forest Park Parent Waiver for Underage Swimmers
By submitting this form, you agree to allow your son or daughter to swim at the Forest Park Pool unattended by myself or any other adult. Only children 12 and over qualify for this status. I will not hold the Lifeguards, Pool Management company or the Forest Park Homeowners Recreation Association (FPHRA) liable for any injuries, disease or harm to my child.
