LIABILITY RELEASE FORMS
Click this link to print the liability release forms for the arena: Knudson Training Stables
Liability Release or open the pdf version that will be attached to your
confirmation email. If you rode at last year’s Back in the Saddle then no need to fill this out again! We’ve got them on file. Please print and sign these forms and then email them
to slohorseproject@gmail.com, text a picture of them to 805-674-5831, or
bring them to the show. As the 4-H Youth Development Program is run by the
University of California at Davis, an additional liability form for the UC will
need signing at the show during registration.
