This form is for case managers to request referrals to service clients in CoC-funded housing programs who are either looking for housing or need help maintaining it. CAFTH staff will not directly refer clients or guarantee housing referrals. Our aim is to assist case managers in identifying the appropriate services to support their clients' housing stability.
