This form is used to sign your child up for Sea Crest's Extended Day Program (EDP) on a contract basis. Please note:
* A separate form must be submitted for each child
* EDP contract rate will be billed to your family's SchoolAdmin billing account on the 1st day of the month
* EDP contract rate will remain in place until you submit another form to notify the School of a cancellation or change
* You do not need to specify which day(s) your child will attend if the 1, 2, 3, 4 or 5 day option is selected
