Thank you for your interest in the LionPulse Smoothie Bike! The Smoothie Bike may only be borrowed by units of The Pennsylvania State University or Penn State Health. Please read over the responsibilities for borrowing the bike listed below and submit the form 30 days prior to the requested event.

* Note: This is simply a borrowing of equipment. Penn State PRO Wellness is happy to suggest recipes, but the person in charge of leasing the equipment is responsible for the demonstration and making of smoothies.

*  The individual borrowing the unit is responsible for transportation of the bike to and from the Academic Support Building, located near the Penn State Health Milton S. Hershey Medical Center in Hershey, PA.

*Requestors will receive a procedural manual for the Smoothie Bike and requestor agrees to read and abide by the manual.

* All materials are to be returned thoroughly cleaned and free of damages.

* When advertising your event, please write the disclaimer "Participation in this event is voluntary. Please check with your physician prior to exercising." on all advertisement pieces.
