SSN Letter Request form
Departments should complete this form when they are hiring an international student to work on campus who needs to apply for a Social Security number (SSN).  Departments should first confirm with the student that they do not already have an SSN.

The Office of International Programs will use the information in this form to issue a letter the student needs to apply for their SSN.  

The SSN card will be mailed to the student once it is issued.  The student should report to you/your department once they have it so that the new hire paperwork can be completed through HR's DocuSign process. International Programs is not involved in that process.  We do not not need to be notified once the student has their SSN, and we do not handle any of the actual new hire paperwork/process.  Our role is just to issue this letter that the Social Security office requires for the SSN application process.
