Complete this form to register your child/children for the First Communion program.

Please note: Your child must have already attended a year of classes in either the Christian Formation Program or St. Alphonsus Grade School in order to be involved in this Parish Sacramental Preparation Program.

There is a $75.00 fee for this program. Payment can be made electronically through WeShare (https://st-alphonsusgr.weshareonline.org/ws/opportunities). We also accept cash or checks (made payable to St. Alphonsus CF).

First Communion Orientation is Sunday, September 29, 2019 at 6 PM in the St. Alphonsus Church - this is a parent/guardian only meeting. Bring the following to the Orientation: a copy of your child's baptism certificate, fee of $75.00 (if not already paid before Orientation).
