Health Promotion Request Form
Thank you for your interest in collaborating with the Health Promotion department. This form is for us to understand and address your needs to the best of our abilities. Please fill out all fields below. You will be contacted within 1-3 business days by the team member(s) assigned to your event.

Please submit the request form at least 2 weeks before the date of the event. If you would like to request a custom program, note that we need a minimum of 4 weeks to design a workshop to best fit your needs. Most of our programming can be offered both in-person or remotely.

If you would like to schedule more than one of our offerings, please fill out a new form for each request.

Health Promotion Services
The Health Promotion Department offers a wide variety of programs for you to choose from:

Educational Workshops: 60 - 90 minute workshops that have been developed and will be presented by well-trained student employees or a professional staff member. These workshops cover a variety of topics designed to directly influence students' academic and personal success. This is great for any type of student gathering (during a class, club meetings, student employee meetings, Residence Halls, etc.)

Tabling Events: We can set up tabling for an event you're hosting or this can be a standalone service. We will set up a table with various information, resources, and activities based on the topic you select as well as general Student Health and Counseling (SHAC) resources.

Project Collaboration: Want to work with us on a specific project? Tell us all about it!

Services Overview: 15 - 45 minute workshops that give an overview of health and wellbeing resources on campus, such as SHAC and the Health Promotion department, as well as mental health services at PSU and the Portland community
