This form will assist you in making a report of sexual discrimination, harassment or violence within the Jessup community. The answers you provide will be used by the Title IX Coordinator to investigate potential sexual discrimination, harassment and/or violence and to provide supportive measures and other responses in accordance with Jessup's Title IX Policy. You may choose to remain anonymous, but please note that Jessup's ability to respond may be limited when your identity and/or other information is withheld.

You are not required to use this form when making a report. You can also make a report to the Title IX Coordinator by voicemail, email or U.S. mail using the following contact information.  The Title IX Coordinator can also answer questions about Jessup's policy and assist you in determining next steps.
