ARPA Connect Case Management Agencies to Admission, Discharge, and Transfer Data Grant

Project Description:

The Department is creating an optional, grant-funded opportunity for interested Case Management Agencies (Single Entry Point Agencies, Community Centered Boards, and Private Case Management Agencies), who serve members enrolled in Home and Community Based Services (HCBS). The optional opportunity, or pilot, supports the provision of a participating Case Management Agency (CMA) with Admission, Transfer, and Discharge (ADT) hospital data for members the agency supports. The Department will provide participating agencies with human-readable Excel files with member ADT information via a secure site. File distribution to be predicated on agency preference regarding cadence. Each CMA and Department staff will work closely together to understand the benefits of receiving timely ADT information.

The ADT Excel files will be uploaded to the Department-managed CMAs’ External SharePoint sites. If a given agency has a strong preference for another access method, please note this preference in the application. The size of the ADT file will vary based on the number of members the agency serves, and could be impacted by the cadence and preference of the CMA. The Department will cleanse the ADT data prior to distribution, with the goal of providing CMAs with well-formatted, accurate, and usable information.

Agencies selected for participation will work closely with the Department to develop a grant agreement with deliverables related to the submitted proposal. Awardees will be required to provide regular reporting and data sharing throughout the grant period, including any challenges that were experienced during the pilot, to include operational (data retrieval and distribution) and programmatic (use of the file to support case management work). Reporting may include Critical Incident Report (CIR) tracking, feedback on the usability of the ADT file, utility in supporting case management work, etc.


Funding Source

The Department was awarded funding through Section 9817 of the American Rescue Plan Act of 2021 (ARPA) to aid the connection of Case Management Agencies (CMAs) to Admission, Transfer, and Discharge (ADT) hospital data. The goal of this project is to improve care coordination and timely critical incident reporting to better support Health First Colorado members. The funds for this grant will be used to support the increased workload of the CMA as a result of participating in this pilot. This is intended to supplement existing funds and cannot be used to supplant existing dollars.


To be eligible for this grant, awardees must meet the following criteria:

  1. Single Entry Point Agencies and Community Centered Boards contracted to provide case management services for Home and Community Based Services (HCBS) with the Colorado Department of Health Care Policy and Financing (Provider Type 11, Specialty 330) and/or

  2. Case Management Agencies that serve children enrolled in the Children’s Home and Community Based Services Waiver (Provider Type 11, Specialty 330: Provider Type 36, Specialty 618)

The Department is looking for diversity in CMAs who participate (varying agency size, rural and urban communities, etc.). All CMAs that meet eligibility requirements are encouraged to apply.

Submission considerations

When submitting applications, please  keep the following in mind:

  • Applications must document how the funding will be used, including clarity on how the funding will supplement existing IT and case management operational costs; funding cannot supplant existing funding for existing agency operations.

  • The Department may not have sufficient funding to support all requests; agencies are encouraged to be as concise, yet clear as possible, in describing the need. 

  • Reimbursement will be made quarterly, pursuant to the provision of appropriate documentation. Applications should outline the type of documentation they intend to provide.
