I give my child permission to participate at the 2017 VBS in the UAD, held at the 1st Presbyterian Church of Michigan City, 121 W. 9th Street. I understand that during the hours of 5:30-7:30 on July 11, 12, and 13, the children will be in the building of 1st Presbyterian Church, safely cared for in a no less than 5-1 ratio, and will also safely walk in a supervised group (with the same ratio) along Franklin street, no further than from 9th to 4th streets. I also give my child permission to participate in the activity on the grounds of Trinity Episcopal church on Thursday, July 13. I understand that my child is to be dropped off at the First Presbyterian Church each night at 5:30, and will be fed dinner. I also understand that my child is to be picked up at the First Presbyterian Church at 7:30 on July 11 and 12, and at TRINITY EPISCOPAL on July 13, and that my family and I are invited to remain past 7:30 for a family celebration. *
