Thank you for taking the time to share your concerns with Lab's Threat Assessment Team. The Threat Assessment Team is available for anyone to share information about any individual who may be planning or is at risk of engaging in violence to self or others.

The Threat Assessment Team will review all referrals and promptly identify the support and intervention mechanisms needed for the individual of concern while keeping our community healthy and safe.

The information provided on this form is CONFIDENTIAL and will be used solely for the purpose of appropriately addressing the reported concern.
