This form is to assist faculty, staff, and students in processing a refund of Billiken Bucks and/or Bonus Flex that they have purchased in the past. The information gathered is solely for the purposes of refunding  the desired account(s). Please allow 1-2 weeks for processing.

**Note:  Bonus Flex refunds will not include the additional percentage (Bonus) which was added when deposited.
**Note:  Funds that have been added as a gift or incentive from a University department cannot be refunded.
