2. MIDLETON RUGBY CLUB - 25th to 27th March 2024 (Towns Park, Midleton)
3. CLONAKILTY SPORTS COMPLEX - 2nd to 4th of April 2024 (Scartagh, Clonakilty, P85 Y009)

Camp Activities Include:
- Football, Fundamental Movement Skills, Athletics, Fun Team Challenges! Special guest, prizes & plenty of FUN!

* Please note: camp participants do NOT have to be registered members of sporting clubs to attend. The Skillz4Sport Easter Camp is fully insured.

As Parent/Guardian, I hereby consent and confirm that I have authority to consent that my daughter(s) may be conveyed (by ambulance, car or other means) to hospital or a doctor for the purpose of medical attention where such is deemed necessary by the Skillz4Sport Easter Camp Staff/Coaches. *
COVID-19 Camp Information for Parents/Guardians
*Please note the following in accordance with government guidelines:

• Parents/Guardians must share information, including awareness as to the signs and symptoms of COVID-19 in an age appropriate manner with their child(ren).

• No person or child(ren) should attend camp if they are unwell or members of their household are showing symptoms consistent with COVID-19.

• If at any time during the camp your child(ren) displays signs or symptoms of COVID-19, as the parent/guardian, the Camp Coordinator will phone you.

• The Camp COVID-19 Supervisor/Camp Coordinator will along with one other adult accompany your child to the medical room, while adhering to the social/physical distancing guidelines.

• The parent/guardian will transport the child home and should consult with their GP by phone of their child’s symptoms.

STEP 3: YOU WILL RECEIVE A 'CONFIRMATION RECEIPT OF PAYMENT EMAIL' FROM ORLAGH ONCE TRANSFER IS COMPLETED (ALLOW UP TO 24 HOURS)
STEP 4: BRING YOUR DAUGHTER(S) TO CAMP (CHECK IN FROM 9.30AM ON FIRST DAY OF CAMP)

 Please note that your daughter(s) will NOT be officially booked into the camp until FULL payment is made (limited spaces available). An email with receipt of payment will be sent to you once payment is processed (allow up to 24 hours for email confirmation).
