DMU Anonymous Reporting Form
This form will allow the university to receive information anonymously in regards to incidents of hate or harassment that have happened to DMU students. The university takes reports of this nature very seriously and there are a number of support and reporting options available to DMU students.
However, De Montfort University understands that not everyone will want to formally report their experiences which is why we have developed this anonymous form. The university will not be able to investigate any reports received via this form, however you telling us about what has happened through this form helps us monitor the prevalence and type of issues DMU students are experiencing in order that we can make better use of resources available and ensure that our training programmes and initiatives are being targeted to the appropriate areas.

Please note this form is limited to DMU students. DMU staff should refer to the guidance in the Dignity at Work policy and Grievance procedure on how to address and report concerns of this nature (include the links listed below). DMU employees and their families may also access free and confidential counselling support and advice on a range of lifestyle challenges via the Health Assured Employee Assistance service who are contactable on 0800 716 017.
