Please fill out this form to log your community service hours. I highly recommend that you log your hours at the end of each month or trimester, so you don't forget to log them!  Also, any hours from the summer of 2019 do count! Please fill this form out for EACH community service activity you have performed (e.g. submit one entry for your hours volunteering at the library and submit another entry for your hours volunteering at the animal shelter).
