Please fill out this form to request a refund or cancellation for a Campus Recreation service or membership*.

Note that not all refunds will be approved, but we will look into each request and you will be contacted either way.  Please see the cancellation & refund policies page for more specific information on fees and stipulations.

*This form will also be utilized as a payroll deduction "opt-out" or adjustment submission (opt out period: 4/16 - 5/15).
