Letter Request Form
This form can be used to request the following types of letters:
  • Enrollment Verification for future semesters OR within first month of current semester
    Enrollment verifications confirm enrollment status (full-time, half-time, etc.) and are commonly provided to loan servicers, insurance companies, etc.
    *Enrollment verification for past semesters and current semester (after the first month) can be downloaded through your SAIL account

  • Jury Duty Letter
    Letter that provides documentation of enrollment at Oakland University for semester during which Jury Duty is required. This letter will not explicitly excuse a student from jury duty. 

  • Letter of Non-Attendance
    Written documentation used for admissions or transfer purposes indicating that a student has not completed any coursework at Oakland University and an academic record is not available. 
This form cannot be used to confirm graduation dates or grade information. The official transcript and diploma are the university's official means of representing this information.
