Family Educational Rights and Privacy Act Waiver *

As an accepted student of Frostburg State University, I authorize Fraternity and Sorority Life and Leadership (FSLL) to review my academic transcripts for the purpose of registering. To be eligible, I understand that Frostburg State University requires a minimum 2.50 grade point average and full-time status of 12 credit hours to participate in recruitment and that some fraternities/sororities may require higher grade point averages. First year students must have a 2.7 high school grade point average, transfer students 12 completed credit hours at an accredited higher education institution.

If I receive an invitation to join an FSL organization, I understand that my grades will be released every semester to: the chapter president, his/her designee, and/or the national office and advisor of the fraternity/sorority for the purpose of academic review by the fraternity/sorority and/or university with the understanding that this information is not to be disclosed for any other purpose than to grade check for the academic monitoring program and application process.

I wish to waive my rights granted to me by the Family Educational Rights and Privacy Act of 1974 and permit FSU to release my academic information in the form of meets or does not meet the standards of Frostburg State University recruitment policies. I understand that I am in no way required to complete this form, but I will have difficulty being accepted into an organization that has grade monitoring checks.

I understand that if I choose to join, there is an academic obligation that must be maintained during my collegiate membership years.
