Daycare Application Form for 2025-2026 Academic Year 
Dear Parents/Guardians,

Please fill out this form for your child(ren), and submit it at the earliest convenience. Kindly complete one registration form per child.

If you are signing up for a day, please sign up 24 hours prior to your visit. Applications received after 3:30PM time will not be accepted. 

Please kindly wait for our confirmation email in return - we will inform you of the earliest possible date.

Please refer to the Daycare Policy and Daycare Calendar for your reference.

Authorized pick-up person: Please be advised parents and siblings of Grade 3 and above are permitted to pick up their child. Please give FULL names of ALL possible pickup person.  If a sibling is picking up your child, please indicate his/her grade. Your child cannot be dismissed if the school is not notified in advance.
