1. Members are required to sign a Liability Waiver and have a valid Juilliard ID to gain access to the facility.

2. Persons under age 16 may not use the facility without prior authorization and signature from a parent/guardian. Special waivers for minors are available from the Office of Residence Life.

3. Guests of Juilliard School faculty, staff, students; alumni of The Juilliard School; students or guests from The School of the American Ballet are not permitted in the facility. The Frederick P. Rose Fitness Center is only for current Juilliard students, faculty, and staff.

4. Members are held financially responsible for damage/vandalism to School property incurred by their own actions which is beyond normal wear and aging. In cases where the damage is intentional or malicious, disciplinary action will result.

5. Theft of any Frederick P. Rose Fitness Center equipment (including but not limited to: free weights, bands, medicine balls, stability balls, etc.) is not tolerated. Members who commit theft will be held financially responsible to repair/replacement of the equipment and will be subject to disciplinary action and indefinite suspension of Fitness Center privileges.

6. Members will be provided an Access Code for the Frederick P. Rose Fitness Center for sole use by that member; providing other individuals with the Access Code is a violation of facility policy and subject to disciplinary action and indefinite suspension of Fitness Center privileges.

**Upon submission of this waiver, you will be emailed the Access Code for the facility.**

7. Closed-toe athletic shoes required. (No sandals or bare feet).

8. Lockers are for day-use only; overnight storage is not allowed.

9. Beverages in spill-proof plastic containers only; glass is not allowed.

10. Food, tobacco, alcohol, and gum are not permitted in the Fitness Center.

11. Re-racking weights, medicine balls, and stability balls after each use is required.

12. Dropping weights on the floor is not permitted.

13. Profanity or suggestive language will not be tolerated. This is a co-ed facility - proper attire and decorum are expected.

14. Limit your cardiovascular workout to less than 30 minutes if others are waiting.

15. Wipe off equipment after each use with the sanitizer towels provided.

16. The Frederick P. Rose Fitness Center is directly monitored by the Office of Residence Life; however, there is not a full-time Fitness Center Monitor on duty regularly. Participation is at your own risk.

17. In case of an emergency, call 911 and contact the 11th Floor Lincoln Center Security Desk at (212) 799-5000, x7441 or 246. Please instruct the Lincoln Center Security Guard to notify the Office of Residence Life at x7400. 

In consideration for receiving permission to utilize the fitness/training facilities provided by The Juilliard School, I, on behalf of myself, my heirs, executors, administrators and assigns, hereby release The Juilliard School, its trustees, officers, employees or agents (hereinafter referred to as RELEASEES) from any and all liability, claims, demands, actions and causes of action arising out of, related to or resulting from my use of the facilities or the equipment provided.

I certify that I am in good physical condition and have not been advised otherwise by a qualified medical professional.  I understand that the use of the facilities and/or participation in its conditioning or fitness programs shall be at my own risk, and Releasees shall not be liable in the event of any loss or personal injury to me or my guest, if any.

I understand that I will be held personally and financially responsible for any damage, vandalism, theft or similar offenses that I knowingly commit in the Fitness Center. I will be held responsible for any violations of Frederick P. Rose Fitness Center policy and may face disciplinary action and/or indefinite suspension from the facility.

I ACKNOWLEDGE AND REPRESENT THAT I have read the foregoing Waiver  and Release, understand it and sign it voluntarily; no oral representations, statements, or inducements, apart from the foregoing written agreement, have been made; I am at least eighteen (18) years of age and fully competent; and I execute this Waiver and Release for full, adequate and complete consideration, fully intending to be bound by the same.  I intend that this Waiver and Release shall be governed by and construed under the laws of the State if New York.
