In this Supplementary Form:
- 'CANDIDATE' means the Girl for whom a place is sought.

- 'APPLICANT' means the Parent of the Candidate.

- The term 'Parent' includes any person who is not a parent of the Candidate but who has parental responsibility (as defined by the Children Act 1989)

- You are strongly advised to provide as much information as you can to support your application.

- There is a section at the end of this form for use if you have insufficient space in any prior section of the form.

- The Common Application Form (CAF) must be submitted and the Supplementary Form should be submitted in order that your application can be considered fully. Completion of the Supplementary Form is not mandatory; however, if one is not received the Governors will not be able to apply their oversubscription admission criteria and the application will be considered under the ‘Other Girls’ category. The Supplementary Form should be completed online using this form or alternatively you can print a paper version of the form from the School website. Please submit the Common Application Form to the authority in which the Candidate resides by 31st October 2023, and submit the online or paper Supplementary Form to the School by 31st October 2023.

- Please ensure that you submit a COPY of the Candidate's Baptism Certificate to the school by 31st October 2023. If a copy of the Baptism Certificate is not available, please give reasons in Section 15 of this form, and provide other written evidence of reception of the Sacrament, from the relevant Parish. If this cannot be provided, again please give reasons in section 15 of this Form.

- We require verification of Mass attendance for at least the past three years.* This should be provided by either the Candidate’s Parish Priest or another Catholic Priest to whom the Candidate is known, using the Priest Reference Form. If it is necessary to obtain this verification from more than one Priest, e.g. because the Candidate has been living in her current parish for less than three years, or regularly attends Mass at more than one parish, further copies of the Priest Reference Form are available from the School or online at www.stphils.org.uk.The original Priest Reference Form/s must be submitted to the School by 31st October 2023.

Our School
St. Philomena's Catholic High School for Girls is a Voluntary Aided school in the Archdiocese of Southwark. It is in the trusteeship of the Catholic Archdiocese of Southwark and was founded by the religious order of the Congregation of the Daughters of the Cross of Liège. The school is conducted by its Governing Body in accordance with its Trust Deed and Instrument of Government and seeks at all times to be a witness to Jesus Christ. The school is run in accordance with the Code of Canon Law promulgated by the Apostolic See and the official teachings of the Roman Catholic Church. The school makes regular use of the Catholic Liturgy and practices during the school year. We ask all parents and students applying for a place here to respect this ethos and its importance to the school community. The school exists primarily to serve the Catholic community and Catholic children always have priority of admission. The Governing Body welcomes all applications, including those from other denominations and faiths who support the religious ethos of the school. We ask all parents applying for a place here to respect this ethos and its importance to the school community. Having consulted with the Local Authority and other admission authorities, the Governors intend to admit into Year 7, in September 2024, up to 234 pupils without reference to ability or aptitude.
