The Seattle School of Theology & Psychology Student Development Committee (SDC) Action Form
*This form is to be used to recommend students for long-term pastoral care, not for concerns that require more immediate attention or intervention*.   If a student is in crisis and needs immediate help please forgo this form and email us directly at academics@theseattleschool.edu.  

Filling out this form directly connects students to someone from the SDC who will reach out to them (unless that student is already engaged in support processes from Student & Academic Services (SAS) and/or Academics).

Best practice is to inform the student about the SDC and their function in connecting students to resources and support. We’ve provided the following disclosure language to aid you in conversation with the student you are referring, so they know why a member of the SDC may contact them:

The Seattle School has a process to support students who may be experiencing difficult circumstances. The Student Development Committee, a team composed of members of Faculty, Academics, and Student & Academic Services, exists to connect students to additional resources, personal support, and pastoral care. I am passing your name along to the Student Development Committee and you are also welcome to reach out directly to a member of Student & Academic Services for conversation or ongoing support.
