Faculty Guide for Submitting IRB Protocols
All researchers must have completed CITI Training prior to collecting data and submitting a research protocol. Please refer to the CITI Accounts and Training information guide for help.

Logging In
Login to Axiom Mentor [https://www.axiommentor.com/login/shibLogin.cfm?i=su]
- Single Sign-On via Microsoft
- Use your SUNet Login (full email address) and password

Submitting a New Protocol
- Choose My Protocols
- Click Create New Protocol
- Complete the Pre-Protocol Diagnostic Survey
- When finished, the Protocol Submission Form will open
Create IRB Protocol
Please provide as much information as possible. This will aid the IRB in its review process and could save time needed to revise your protocol.
Note: If you cancel before saving your protocol, all information will be lost.
Answer all questions in the Application Sections. Look for the green arrow near the bottom of the protocol home page.

Upload PDF or Word copies of required forms, as applicable. Most protocols require, at minimum, and Informed Consent form. Only SU IRB informed consent templates are accepted; please do not create your own informed consent form. Download and edit the SU informed consent/assent templates found on the SU IRB website [https://www.su.edu/academics/academic-resources-information/irb/] under IRB Forms.

Faculty Advisor’s Review and Acceptance of Student Protocol
Open the IRB page
- Click Student Protocols
- Choose the protocol by clicking on its title on the right-hand side of the list
- Click on supporting document to review the study details
- When ready, Click Faculty Advisor Action at the top of the page and choose your action form the drop-down menu
- Click Update Status when ready
